Appointment Scheduling and Cancellations

Appointments: To schedule an appointment, please call us at 905-674-6477 or use our online booking system. We encourage you to book your appointments in advance to secure your preferred time.

Cancellations: If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice. Cancellations within 24 hours or missed appointments may incur a cancellation fee.

Late Arrivals: If you are running late for your appointment, please call the clinic. We will do our best to accommodate you, but depending on the schedule, we may need to reschedule your appointment.

Payment and Insurance

Payment: Payment for services is due at the time of your appointment. We accept all major credit cards, debit cards, and cash.

Insurance: DECA Health & Wellness works with various insurance providers. Please verify your coverage prior to your appointment. Co-pays, deductibles, and out-of-pocket expenses are the responsibility of the patient.

Self-Pay: If you do not have insurance or prefer to pay out of pocket, we offer competitive pricing and flexible payment options. Please ask our staff for more details.

Confidentiality and Privacy

HIPAA Compliance: We adhere to all HIPAA (Health Insurance Portability and Accountability Act) regulations to ensure your personal and medical information is protected. All patient records are kept confidential and only shared with authorized personnel, unless written consent is provided.

Data Security: Our clinic uses secure systems for scheduling, billing, and medical records to protect your information.

Treatment and Patient Responsibilities

Initial Consultation: All new patients are required to complete an intake form and provide a thorough health history. This allows our practitioners to create a personalized treatment plan.

Treatment Plans: Treatment plans are customized to meet each patient’s needs and goals. It is essential that you follow the recommendations of your therapist for optimal results.

Patient Responsibilities: Patients are expected to actively participate in their treatment plans, including following home exercises or lifestyle modifications, as directed by their therapist.

Conduct and Behaviour

Respectful Behaviour: DECA Health & Wellness maintains a professional and respectful environment. We expect all patients and visitors to treat our staff and other clients with courtesy and respect.

Drug-Free Environment: For the safety and well-being of all, the use of alcohol, drugs, or other substances on the clinic premises is strictly prohibited. We maintain a drug-free workplace in accordance with local regulations.

Hygiene and Safety

Personal Hygiene: Please ensure that you arrive for your appointment clean and dressed in appropriate clothing for your therapy. For massage therapy, comfortable, loose-fitting attire is recommended.

Infection Control: Our clinic follows strict hygiene protocols, including sanitizing treatment areas and equipment between patients. If you are feeling ill or experiencing symptoms of infection, please reschedule your appointment and inform us as soon as possible.

Patient Feedback and Complaints

We Value Your Feedback: Your experience is important to us. If you have suggestions or concerns regarding your treatment or service, please speak with our clinic manager or submit your feedback in writing to info@decahealthandwellness.com

Complaint Resolution: We are committed to resolving any issues fairly and promptly. If you are dissatisfied with your care, please contact our clinic management team to discuss the matter.

Emergency and After-Hours Care

Emergency Care: DECA Health & Wellness does not provide emergency medical services. In case of an emergency, please call 911 or visit the nearest emergency room.

​

After-Hours: Our clinic operates during regular business hours. For after-hours inquiries, leave a message on our voicemail or contact us via email. We will respond on the next business day.

​

Minor Patients

Parental Consent: For patients under 18 years of age, a parent or guardian must be present for the initial consultation and provide consent for treatment.

​

Supervision: A parent or guardian must remain on the clinic premises for the duration of the appointment for any minor patient receiving treatment.
